Before Your Visit
To ensure a smooth and efficient first appointment, please bring any dental insurance information you may have, along with your completed New Patient Forms. This paperwork is crucial as it helps us better understand you and your dental history. If you’ve had dental X-rays taken at another office within the last year, please forward them to us for review. This information will assist us in providing you with the most comprehensive care possible.
During Your Visit
Your initial visit is fundamental to establishing your dental health baseline. We will conduct thorough dental X-rays to examine the current state of your teeth and identify any hidden issues. Our team will also check for signs of cavities or damage, ensuring your teeth are clean, polished, and healthy. We’ll discuss any necessary treatment plans tailored specifically for you after the examination. This is a great time to address any questions or concerns, so don’t hesitate to ask. We aim to ensure you leave well-informed and confident about your dental health plan.
After Your Visit
As your visit concludes, we encourage you to schedule any necessary follow-up appointments as discussed with your dentist. Regular dental checkups and cleanings are pivotal in maintaining oral health, and we generally recommend visits every 3-6 months, depending on your specific needs. This proactive approach ensures that potential issues are addressed promptly and helps maintain your smile’s health and brightness.

Financial Arrangements
At Blue Quill Dental we bill directly to your insurance policy, however you are responsible for the co-payment on the day of the service provided. For your convenience we accept payment by Visa, MasterCard, Debit and Cash. We do not accept Amex at this time.
New Patient Forms
By filling out the forms ahead of time you will save significant time on your visit. If you have any questions when filling out the form, please do the best you can, and our staff will assist you with your questions on the day of your appointment.
These forms can be downloaded to your computer. Please complete the information and bring them with you to your first visit. You may also fill out the forms electronically, save and send the completed PDF files to: admin@bluequilldental.com
